Clearly she is smoking marijuana out of an apple. All you must do is pierce a hole through an apple using something long but not too thick. The hole must go all the way through the apple, one end of the hole directly where the stem is connected to the apple and the other end of the hole anywhere on the side. Basically we can see that Charlize Theron is using an apple as a pipe.
